Does this only apply if Kioko is already assigned to the battlefield ? So if she were to assign in cavalry maneuvers segment after infantry the reaction would not trigger ?

Correct. If she were to Assign as an Infantry Attacker, any assignment defending her would be a trigger for her Reaction.
