The question: It says that your opponent must focus at least once from their hand. But can they focus first from the top of their deck before deciding which card to focus from their hand?
Obviously, if true then they can only choose to do that 3 times before choosing a cad from hand as 4 cards is the limit of cards to be focused and 1 has to come from their hand.

They may.
