Or is it only actions with a single effect of "Ranged x Attack"? If the prior, is it only the Ranged Attack effect which has its targeting restricted by Koshin Keep, or is it all targeting of the Battle action? Am I unable to even play Blood Wave as a Koshin Keep effect additional action because I may not target my own personality or follower to pay the cost of "destroy your target Personality or Follower"?

A Ranged Attack action is one that produces a Ranged Attack as one of its effects.

We have decided that it restricts all targeting of the Battle Action, as in previous rulings.

Yes, I'm afraid you can't use Blood Wave as an additional action granted by Koshin Keep because the choice restriction clashes with the mandatory targeting in Blood Wave.
